Questionnaire: Evaluation of a
Remote Wheelchair Selection-Advisor (RWS-A)

The Rehabilitation Engineering Research Center on Telerehabilitation (RERC-TR) funded by the National Institute on Disability and Rehabilitation Research (NIDRR) of the U.S. Department of Education, is currently conducting a research project to develop a computer-aided resource known as the Remote Wheelchair Selection Advisor (RWS-A). This tool is intended to assist practitioners and suppliers in the selection of wheeled mobility interventions based on the new Medicare coding structure for powered mobility devices.

We invite you to view a short demonstration of the system. The presentation format uses MediaSite Live. If you have difficulty viewing the presentation, please review the system requirements. Also note that some firewalls might prohibit access to the presentation.

This questionnaire is to survey your initial opinion and acceptance of the RWS-A prototype in order to assist with further development. After watching the video, please complete this online survey regarding your opinion of the usefulness of RWS-A. We would also like to collect some basic demographic data about you as a participant. None of this data will be used to identify you specifically but rather to provide us with information regarding trends as to the potential usefulness and usability of the tool therefore the anonymity of your response is assured.

The questionnaire measures your response on a scale of 1 to 7. If you strongly agree with a question, then select your answer with number seven. If you strongly disagree with a question, then mark your answer with number one. You can mark number five or six according to the extent to which you agree with each question. You can mark number two or three as well, according to the extent which you disagree with each question. You cannot choose multiple responses for each question.

The term “task” in the questions refers to your role in selecting an appropriate wheeled mobility device for a particular client.
